Medication-related bradycardia is a significant concern in geriatric cardiology due to polypharmacy and age-related changes in drug metabolism. Common causative medications include beta-blockers, calcium channel blockers, digoxin, and certain antiarrhythmic drugs. Elderly patients are particularly susceptible due to decreased hepatic and renal clearance, altered drug distribution, and increased receptor sensitivity. The condition may present with fatigue, dizziness, syncope, or cognitive impairment, symptoms that may be attributed to other age-related conditions. Management involves careful medication review, dose adjustment, or drug discontinuation while balancing the cardiovascular benefits of the medications. In some cases, pacemaker implantation may be necessary to allow continuation of essential cardiac medications.
