Transcatheter implantation of a new valve within a previously implanted prosthetic valve, commonly performed in elderly patients with degenerated bioprostheses.

Full Definition

Valve-in-valve (ViV) procedures involve the transcatheter placement of a new valve within a failing or degenerated prosthetic valve, typically a bioprosthetic valve. This technique is particularly relevant in geriatric cardiology as elderly patients with previously implanted bioprosthetic valves may experience structural valve deterioration over time. The procedure offers a less invasive alternative to redo cardiac surgery, which carries significantly higher risk in elderly patients. ViV procedures can be performed for aortic, mitral, tricuspid, or pulmonary positions, with aortic ViV being most common. Considerations include the size and type of the original prosthesis, the mechanism of failure, and the potential for patient-prosthesis mismatch.
