Pouch of Douglas obliteration refers to the complete or partial closure of the rectovesical pouch (pouch of Douglas), the most dependent part of the female peritoneal cavity located between the posterior uterus and anterior rectum. This condition is most commonly caused by severe endometriosis, but can also result from pelvic inflammatory disease, previous surgery, or malignancy. Obliteration can be partial or complete and significantly impacts surgical approaches, particularly during hysterectomy or endometriosis treatment.
