Full Definition

The uterosacral ligaments are fibromuscular structures that form part of the cardinal ligament complex, providing crucial support to the uterine cervix and upper third of the vagina. These paired ligaments contain smooth muscle, connective tissue, and autonomic nerves, extending posteriorly from the cervix and upper vagina to insert on the sacrum at the level of S2-S4. They are frequently involved in pelvic organ prolapse and may be surgically modified during procedures such as uterosacral ligament suspension. Injury to these structures during surgery can result in pelvic organ prolapse or denervation.
