Full Definition

The extensor carpi radialis longus (ECRL) is a muscle of the posterior forearm that originates from the lateral supracondylar ridge of the humerus and inserts into the base of the second metacarpal. It functions primarily to extend and radially deviate the wrist and is innervated by the radial nerve. The ECRL is clinically significant as it is commonly used as a donor tendon for various reconstructive procedures, particularly in patients with radial nerve paralysis or spinal cord injuries. It provides strong wrist extension and is often transferred to restore finger and thumb function. The muscle-tendon unit has good excursion and strength, making it valuable for tendon transfer procedures. In lateral epicondylitis, the ECRL may be involved in the pathological process affecting the common extensor origin.
