The Mayfield classification describes four progressive stages of perilunate instability based on the predictable pattern of ligamentous disruption that occurs with wrist trauma. Stage I involves scapholunate dissociation, Stage II adds capitolunate disruption, Stage III includes lunotriquetral dissociation (complete perilunate dislocation), and Stage IV represents lunate dislocation with complete disruption around the lunate.
