The anterolateral thigh (ALT) flap is a versatile microvascular free tissue transfer harvested from the lateral aspect of the thigh, based on perforators from the descending branch of the lateral circumflex femoral artery. It provides a large amount of skin and subcutaneous tissue with a reliable vascular pedicle, making it ideal for reconstruction of extensive head and neck defects following oncologic resection. The flap can be harvested as a fasciocutaneous or myocutaneous flap and offers minimal donor site morbidity compared to other free flap options.
