Arytenoidectomy is a surgical procedure involving the partial or complete removal of one or both arytenoid cartilages to enlarge the posterior glottic airway. This technique is primarily indicated for patients with bilateral vocal cord paralysis who have inadequate airway caliber despite tracheostomy decannulation attempts. The procedure can be performed endoscopically using laser or microdebrider techniques, or through an open approach. Arytenoidectomy is often combined with posterior cordectomy or vocal cord lateralization to maximize airway improvement while attempting to preserve voice quality. Post-operative complications may include aspiration, voice changes, and granulation tissue formation.
