Full Definition

Field cancerization refers to the molecular and histologic changes that occur throughout the mucosa of the upper aerodigestive tract following exposure to carcinogens such as tobacco and alcohol. This concept, first described by Slaughter in 1953, explains how apparently normal-appearing mucosa adjacent to tumors may harbor genetic alterations that predispose to malignant transformation. In head and neck oncology, field cancerization accounts for the high incidence of second primary tumors, local recurrences at surgical margins that appear histologically clear, and the challenges in achieving complete tumor eradication. Understanding this concept is crucial for treatment planning and surveillance strategies.
