A complex reconstructive procedure involving the transfer of tissue (skin, muscle, bone, or combinations) from a distant anatomical site with complete disconnection and subsequent microvascular reattachment of blood vessels. Common donor sites include the fibula for mandibular reconstruction, radial forearm for soft tissue coverage, and anterolateral thigh for large defects. The success of free flap reconstruction depends on microsurgical expertise and careful postoperative monitoring for vascular compromise.
