Modified radical neck dissection removes all lymph node levels (I-V) while preserving at least one of three key structures: the sternocleidomastoid muscle, internal jugular vein, or spinal accessory nerve. This approach maintains oncologic adequacy while reducing functional morbidity compared to radical neck dissection. The specific preserved structures must be documented clearly, as this affects staging and prognosis. Different classifications exist based on which structures are spared.
