radial forearm free flap

Microvascular free flap based on the radial artery, commonly used for oral cavity reconstruction. Also known as the Chinese flap.

Full Definition

A versatile microvascular free flap consisting of skin and subcutaneous tissue from the volar aspect of the forearm, based on the radial artery and its venae comitantes. First described by Yang et al. in China, it has become a workhorse flap for reconstruction of oral cavity defects due to its thin, pliable tissue that closely matches oral mucosa. The flap provides reliable vascularity with a long pedicle and large-caliber vessels, making microsurgical anastomosis technically favorable. It can be harvested as a fascial, fasciocutaneous, or osteofasciocutaneous flap when combined with a segment of radius bone. Primary indications include reconstruction of tongue, floor of mouth, buccal mucosa, and soft palate defects. The main disadvantage is the visible donor site scar on the forearm, which patients must accept. Preoperative Allen's test is essential to confirm adequate ulnar circulation before harvest.
