Blinatumomab is a novel immunotherapy that simultaneously binds to CD19 on malignant B cells and CD3 on T cells, creating an immunological synapse that activates T-cell-mediated cytolysis of leukemic cells. It is administered as a continuous intravenous infusion over 28-day cycles for relapsed/refractory B-cell acute lymphoblastic leukemia. The drug has shown remarkable efficacy in achieving complete remissions in heavily pretreated patients. Significant toxicities include cytokine release syndrome and neurologic events, requiring intensive monitoring and specialized management protocols.
