Daratumumab is a human monoclonal antibody that targets CD38, a glycoprotein highly expressed on multiple myeloma cells. It works through multiple mechanisms including antibody-dependent cellular cytotoxicity, complement-dependent cytotoxicity, and antibody-dependent cellular phagocytosis. The drug is used in both newly diagnosed and relapsed/refractory multiple myeloma settings, often in combination with other agents such as lenalidomide, bortezomib, or carfilzomib. Infusion reactions are common with the intravenous formulation, requiring premedication and monitoring. The subcutaneous formulation has reduced infusion reaction rates and shorter administration times.
