The prozone effect, also known as the hook effect in two-site immunoassays, occurs when extremely high concentrations of the target hormone saturate both capture and detection antibodies, preventing the formation of proper antibody-antigen-antibody complexes. This results in paradoxically low or even negative results despite very high actual analyte concentrations. In hormone testing, prozone effects are most commonly encountered with protein hormones like prolactin, hCG, and thyroglobulin when concentrations are exceptionally elevated due to tumors or pathological conditions. Recognition of this phenomenon is crucial for laboratory professionals, as it can lead to serious diagnostic errors if not detected.
