The contrast volume to creatinine clearance ratio (CV/CrCl) is a validated risk prediction tool used to assess the likelihood of contrast-induced acute kidney injury (CI-AKI) following coronary angiography and intervention. This ratio is calculated by dividing the total volume of iodinated contrast agent administered (in milliliters) by the patient's estimated creatinine clearance (in mL/min). Ratios exceeding 3.7 are associated with significantly increased risk of CI-AKI, particularly in patients with baseline renal impairment or diabetes. This metric helps guide contrast volume limitations during procedures and informs decisions regarding prophylactic measures such as pre- and post-procedural hydration, nephrotoxic medication withholding, and consideration of alternative imaging strategies.
