This method involves accessing the contralateral femoral artery and advancing catheters across the aortic bifurcation to reach the target coronary vessels. The technique is particularly valuable in patients with severe peripheral artery disease, previous vascular complications, or anatomical variants that preclude direct access. Crossover requires longer, specialized equipment and careful attention to catheter torque transmission across the bifurcation. While technically more challenging, this approach enables successful coronary intervention in patients who might otherwise require surgical revascularization due to access limitations.
