Proximal optimization technique (POT) is a specialized method for optimizing stent deployment in the proximal main vessel segment of bifurcation lesions. The technique involves inflating a short, appropriately sized balloon from the main vessel ostium to the level of the carina, correcting stent underexpansion and malapposition that commonly occurs in the proximal segment due to vessel tapering and geometric complexity. POT improves stent scaffolding, facilitates side branch access if needed, and reduces the risk of stent thrombosis by ensuring optimal strut apposition. This technique is typically performed after main vessel stenting and before any side branch intervention, and may be repeated after side branch treatment to restore optimal main vessel geometry.
