Side branch compromise refers to the reduction in luminal diameter, flow, or complete occlusion of a side branch vessel that occurs as a consequence of main vessel stenting or balloon angioplasty, especially in bifurcation lesions. This complication can result from plaque shift, dissection propagation, or mechanical obstruction by stent struts or displaced atherosclerotic material. The clinical significance depends on the size of the affected side branch, the presence of collateral circulation, and the territory of myocardium supplied. Management strategies include balloon angioplasty through stent struts, side branch stenting, or acceptance of the compromise if the vessel is small and the territory is adequately collateralized. Prevention techniques include provisional stenting strategies and careful lesion preparation.
