A rare but potentially fatal complication where CO2 gas enters the vascular system during laparoscopic insufflation, causing circulatory obstruction.

Full Definition

Gas embolism is a serious complication of laparoscopic surgery that occurs when carbon dioxide used for pneumoperitoneum inadvertently enters the patient's vascular system. This can happen through direct vascular injury during trocar insertion, inadvertent intravascular injection, or through opened venous channels during dissection. Once in the circulation, gas bubbles can cause mechanical obstruction of blood flow, particularly dangerous when affecting coronary or cerebral circulation. Clinical presentation may include sudden cardiovascular collapse, arrhythmias, hypotension, or neurological symptoms. Prevention involves careful trocar insertion techniques, maintaining appropriate insufflation pressures, and prompt recognition of vascular injury. Treatment requires immediate desufflation, patient positioning (left lateral Trendelenburg), high-flow oxygen, and potentially hyperbaric oxygen therapy. Early recognition and aggressive management are crucial for patient survival.
