An iatrogenic perforation or thermal injury to the rectal wall occurring during laparoscopic procedures, most commonly during deep pelvic dissection or endometriosis surgery. Recognition may be immediate through visualization of rectal mucosa or delayed through postoperative symptoms. Management depends on the size and location of injury, ranging from primary laparoscopic repair to temporary diverting colostomy. Risk factors include severe endometriosis, previous pelvic surgery, inflammatory conditions, and anatomical distortion.
