The biophysical profile (BPP) is a comprehensive prenatal test that combines real-time ultrasound assessment with non-stress test monitoring to evaluate fetal well-being. The test scores five parameters: fetal breathing movements, gross body movements, fetal tone, reactive fetal heart rate (non-stress test), and amniotic fluid volume. Each parameter receives 0 or 2 points, with a maximum score of 10. A score of 8-10 is considered normal, 6 is equivocal and may require repeat testing, and scores of 4 or less indicate potential fetal compromise requiring immediate evaluation for delivery. The BPP is commonly used for high-risk pregnancies and when other surveillance tests are abnormal.
