In psychiatric documentation, a patient's awareness and understanding of their own mental illness and its effects on their behaviour and functioning.

Insight is a standard domain assessed in the mental status examination and is typically rated on a spectrum: intact (full awareness of illness), partial (some recognition), or absent (no acknowledgement of illness). Editors should note that 'insight' in psychiatric records carries a specific clinical meaning distinct from its everyday usage and should not be substituted with words such as 'self-awareness' or 'understanding' without the clinician's approval. Poor insight is clinically significant as it often correlates with medication non-adherence and poorer prognosis. It is frequently paired with 'judgment' as a paired assessment: 'insight and judgment are intact/impaired.'
