Wraparound Services

A holistic, community-based service delivery model that coordinates multiple supports around an individual and their family to address mental health and related needs.

Full Definition

Wraparound services is a care-coordination philosophy and practice model originating in children's mental health systems, now used across age groups, that convenes a team of formal providers and natural supports to build a strengths-based, individualised plan. In documentation, it appears in case management notes, county mental health plans, and Medicaid waiver applications. Editors should note the compound noun is typically written as two unhyphenated words ('wraparound services') when used as a noun phrase, but is hyphenated when used as a compound modifier ('wraparound-services coordinator'). It should not be confused with WRAP (Wellness Recovery Action Plan), a distinct self-management tool.
