preperitoneal space

Anatomical space between the peritoneum and abdominal wall muscles used in some laparoscopic procedures.

Full Definition

The preperitoneal space, also known as the space of Retzius, is the extraperitoneal area located between the posterior surface of the abdominal wall muscles and the anterior peritoneum. In laparoscopic surgery, this space can be accessed and insufflated with CO₂ to perform totally extraperitoneal (TEP) procedures, particularly for inguinal hernia repairs. Working in the preperitoneal space avoids entering the peritoneal cavity, reducing the risk of bowel injury and adhesion formation. The space contains loose areolar tissue and allows visualization of important anatomical landmarks such as the inferior epigastric vessels and Cooper's ligament. Proper identification of this space is crucial for successful extraperitoneal laparoscopic techniques.
