The KREC assay (Kappa-deleting Recombination Excision Circles assay) is a quantitative PCR test that measures DNA excision circles produced during B-cell development. This assay serves as a biomarker for B-cell lymphopenia and certain primary immunodeficiency disorders. KRECs are stable circular DNA molecules formed when immunoglobulin genes rearrange during B-cell maturation in bone marrow. Low KREC levels may indicate disorders affecting B-cell development, such as X-linked agammaglobulinemia or other forms of severe combined immunodeficiency. The test is often performed alongside TREC assays to provide comprehensive assessment of both T-cell and B-cell immunity in newborns.
