Triad of head tilt, skew deviation, and ocular torsion resulting from otolith pathway dysfunction in the brainstem.

Full Definition

The ocular tilt reaction (OTR) is a stereotyped response consisting of conjugate eye torsion, skew deviation, and compensatory head tilt. This triad results from disruption of otolith pathways in the brainstem, particularly affecting the vestibular nuclei or their connections. The reaction represents the brain's attempt to maintain visual stability when gravitational input is disrupted. OTR can be caused by various brainstem lesions, including stroke, demyelination, or tumors. The direction of the head tilt and skew deviation indicates the side of the lesion. Recognition of OTR is important for localizing brainstem pathology and may be the only sign of a small brainstem infarct.
