Wall-Eyed Bilateral INternuclear Ophthalmoplegia syndrome, characterized by bilateral medial longitudinal fasciculus (MLF) lesions causing bilateral internuclear ophthalmoplegia with an associated exotropia. Patients present with impaired adduction bilaterally, convergence insufficiency, and a wall-eyed appearance due to the exotropia. Most commonly caused by demyelinating disease such as multiple sclerosis, but can result from brainstem strokes or tumors. The acronym emphasizes the characteristic outward deviation of both eyes.
