Small, old stroke in deep brain structures appearing as fluid-filled cavity with surrounding gliosis.

Full Definition

Chronic lacunar infarcts are small, chronic strokes typically occurring in the deep white matter, basal ganglia, or brainstem due to occlusion of small penetrating arteries. These lesions appear as small cystic spaces (usually <15mm) with signal characteristics similar to cerebrospinal fluid on all MRI sequences. They are surrounded by a rim of gliosis and may show minimal mass effect. Unlike acute lacunar infarcts, chronic lesions do not restrict diffusion and often have well-defined borders. They are associated with hypertensive small vessel disease and diabetes, and multiple lacunes may contribute to vascular cognitive impairment.

Usage note: Specify location and number, as multiple lacunes suggest systemic small vessel disease.
