Needle electrode examination is the component of electromyography that involves inserting needle electrodes into muscles to directly record electrical activity. The examination includes assessment of insertional activity, spontaneous activity at rest, and voluntary motor unit potentials during muscle contraction. Different types of abnormal spontaneous activity include fibrillations, positive sharp waves, fasciculations, and complex repetitive discharges, each with distinct diagnostic implications. The examination also evaluates motor unit potential morphology, recruitment patterns, and interference patterns. This technique is essential for diagnosing myopathies, neuropathies, neuromuscular junction disorders, and motor neuron diseases.
