Pudendal nerve terminal motor latency (PNTML) is a specialized nerve conduction study that measures the conduction time in the distal portion of the pudendal nerve from the ischial spine to the external anal sphincter. The test uses a specialized electrode mounted on the examiner's finger to stimulate the nerve transrectally while recording from the external anal sphincter. PNTML is used to evaluate pudendal nerve function in patients with fecal incontinence, chronic pelvic pain, and other pelvic floor disorders. Normal values are typically less than 2.2 milliseconds, with prolonged latencies suggesting pudendal neuropathy.
