Corticotrophs are specialized endocrine cells comprising approximately 15-20% of the anterior pituitary cell population, responsible for the synthesis, storage, and regulated secretion of adrenocorticotropic hormone (ACTH) and related pro-opiomelanocortin (POMC)-derived peptides including β-endorphin. These cells express the POMC gene and respond to hypothalamic corticotropin-releasing hormone (CRH) and vasopressin through specific G-protein coupled receptors, leading to increased cAMP and ACTH release. Corticotrophs are subject to negative feedback regulation by glucocorticoids and play a central role in the hypothalamic-pituitary-adrenal stress response axis. Corticotroph adenomas can cause Cushing's disease, while destruction of these cells leads to secondary adrenal insufficiency.
