Pro-opiomelanocortin (POMC) is a large precursor protein primarily synthesized in the anterior pituitary corticotrophs and hypothalamic arcuate nucleus neurons. Through tissue-specific post-translational processing by prohormone convertases, POMC gives rise to multiple bioactive peptides with diverse functions. In the anterior pituitary, POMC processing yields adrenocorticotropic hormone (ACTH), β-lipotropin, and β-endorphin, crucial for stress response and pain modulation. In hypothalamic neurons and intermediate pituitary, additional processing produces α-melanocyte-stimulating hormone (α-MSH) and other melanocortin peptides that regulate feeding behavior, energy expenditure, and pigmentation. POMC deficiency causes severe early-onset obesity, adrenal insufficiency, and red hair pigmentation due to the loss of these critical regulatory peptides.
