Paddle electrodes are flat, rectangular electrode arrays designed for surgical placement directly against the spinal cord in the epidural space. Unlike cylindrical percutaneous leads, paddle electrodes require laminectomy for placement but offer advantages including multiple contact configurations, broader coverage areas, and greater stability once implanted. They are typically reserved for patients requiring extensive stimulation coverage or those who have failed percutaneous approaches.
