Akinetic mutism represents the most severe form of abulia, characterized by a wakeful but unresponsive state where patients can track with their eyes but fail to initiate voluntary movements or speech. This condition typically results from bilateral lesions to the anterior cingulate cortex, supplementary motor area, or mesial frontal regions. Unlike coma or vegetative states, patients retain awareness and can sometimes respond to vigorous stimulation. The condition represents a disconnect between intention and action execution.
