Effort testing, also known as performance validity testing, involves the systematic assessment of whether examinees are putting forth adequate effort during neuropsychological evaluation. These measures help identify poor effort, whether intentional (malingering) or unintentional (due to factors like depression, fatigue, or lack of understanding). Effort tests are designed to be so easy that even individuals with significant cognitive impairment should pass them when trying their best. Common effort measures include forced-choice recognition paradigms, measures of response consistency, and tests that capitalize on cognitive patterns unlikely to occur with genuine impairment. Results inform the validity and interpretability of other test findings, with poor effort potentially invalidating an entire assessment.
