Topographical disorientation is a spatial cognitive disorder characterized by severe difficulty in navigating through familiar environments, learning new spatial layouts, or maintaining spatial orientation during travel. This condition can manifest as inability to recognize familiar landmarks, difficulty forming cognitive maps of spaces, problems with route learning, or loss of directional orientation. Topographical disorientation typically results from lesions in the right hemisphere, particularly involving the posterior parietal cortex, retrosplenial cortex, or hippocampal formation. The condition can significantly impact independence and quality of life, as patients may become lost in previously familiar locations like their neighborhood or even their own home. Assessment involves testing landmark recognition, route learning, and spatial memory using both verbal report and actual navigation tasks.
