Carboprost tromethamine (Hemabate) is a synthetic prostaglandin F2α analog used as a second-line uterotonic agent for postpartum hemorrhage when oxytocin is ineffective. It causes powerful uterine contractions and vasoconstriction but has significant side effects that impact anesthetic management. These include bronchospasm (contraindicated in asthma), pulmonary vasoconstriction, systemic hypertension, and gastrointestinal effects like nausea, vomiting, and diarrhea. The medication can also cause hyperthermia and should be used cautiously in patients with cardiovascular disease. Typical dosing is 250 μg intramuscularly, which can be repeated every 15-90 minutes.
