A fetal scalp electrode (FSE) is an internal fetal monitoring device that consists of a spiral wire electrode that is attached directly to the fetal scalp through the cervix to obtain continuous fetal electrocardiogram signals. This method provides more accurate and reliable fetal heart rate monitoring compared to external cardiotocography, particularly in cases of maternal obesity, fetal movement, or poor external signal quality. In obstetric anesthesia, FSE monitoring becomes especially valuable when assessing fetal response to neuraxial procedures, as it eliminates signal artifacts that might mask important fetal heart rate changes. The placement requires ruptured membranes and adequate cervical dilation, and the electrode must be removed before delivery to prevent fetal injury.
