The highest level of sympathetic nerve blockade achieved during neuraxial anesthesia. Typically extends 2-6 dermatomes higher than sensory blockade and determines cardiovascular effects.

Sympathectomy level refers to the uppermost extent of sympathetic nervous system blockade produced by neuraxial anesthesia, which typically extends 2-6 dermatomes higher than the corresponding sensory (pinprick) level. This differential blockade occurs because sympathetic nerve fibers are smaller and more susceptible to local anesthetic blockade than sensory nerve fibers. In obstetric anesthesia, the sympathectomy level is clinically significant because it determines the degree of cardiovascular effects, particularly hypotension and bradycardia. A sympathectomy level above T4 can block cardioaccelerator fibers, leading to significant bradycardia, while extensive sympathectomy can cause profound hypotension due to peripheral vasodilation and decreased venous return. Understanding and monitoring sympathectomy levels helps predict and manage hemodynamic changes during cesarean section under spinal or epidural anesthesia.
