Anisocoria refers to a difference in pupil diameter between the two eyes, with the disparity typically measured in millimeters under standardized lighting conditions. While mild anisocoria (≤1mm) can be physiological and benign, significant differences may indicate serious conditions such as Horner's syndrome, third nerve palsy, or pharmacological pupil dilation. In the perioperative setting, anisocoria can result from surgical trauma, medications, or complications affecting pupillary function. Proper documentation includes pupil sizes in both light and dark conditions to characterize the underlying mechanism.
