Explantation

Surgical removal of a previously implanted intraocular device, typically an IOL or glaucoma drainage device.

Full Definition

Explantation in ocular surgery refers to the surgical removal of previously implanted devices, most commonly intraocular lenses (IOLs) or glaucoma drainage implants. IOL explantation may be necessary due to incorrect lens power, IOL dislocation, chronic inflammation, or lens design-related complications. The procedure can be technically challenging, particularly with calcified or well-integrated implants, and may require specialized techniques or instrumentation. Glaucoma device explantation is typically performed for infection, erosion, or device failure. Successful explantation often requires careful planning to minimize complications such as vitreous loss, capsular rupture, or tissue damage, and may necessitate concurrent procedures such as IOL exchange or additional glaucoma surgery.

Usage

Usage note: Distinguish from IOL exchange procedures where a new lens is immediately implanted; explantation may leave the eye aphakic.
