A fornix-based flap is a surgical approach used in glaucoma filtration surgery where the initial conjunctival incision is made at the fornix (the fold where the conjunctiva reflects from the eyelid to the eyeball) and the flap is created toward the limbus. This technique contrasts with limbus-based flaps and is thought by some surgeons to provide better wound healing and reduced scarring in trabeculectomy procedures, though both approaches have their advocates in current practice.
