A standardized surgical approach for rectal cancer that involves sharp dissection along embryologic fascial planes to remove the rectum and its surrounding mesorectal fat as an intact unit. Total mesorectal excision (TME) has become the gold standard for mid and low rectal cancers, significantly reducing local recurrence rates. Editors should be aware that partial mesorectal excision (PME) is appropriate for upper rectal cancers.
