Sclerosing polycystic adenosis (SPA) is a poorly understood benign condition of the major and minor salivary glands, most commonly affecting the parotid gland. It is characterised histologically by lobular proliferation of acinar and ductal elements within a sclerotic stroma, with frequent apocrine metaplasia and cyst formation. The aetiology remains unclear, and the lesion may be reactive or represent a true neoplasm. Despite its benign nature, local recurrence has been reported in a subset of cases. Editors should be aware that the term has evolved over time and older literature may use variant nomenclature.
