Maxillomandibular advancement (MMA) is a surgical procedure that involves simultaneous advancement of both the maxilla and mandible to increase the dimensions of the upper airway for treatment of obstructive sleep apnea. The procedure typically combines Le Fort I osteotomy for maxillary advancement with bilateral sagittal split osteotomy for mandibular advancement. By moving both jaws forward, the attached soft tissues including the tongue base are also advanced, creating more space in the pharyngeal airway. This procedure is considered the most effective surgical treatment for OSA, with success rates often exceeding 90% in properly selected patients. MMA is typically reserved for patients with severe sleep apnea who have failed other treatments and have adequate facial projection to tolerate the advancement.
