The pterygomaxillary fissure is a vertical cleft located between the posterior wall of the maxilla and the anterior border of the pterygoid process of the sphenoid bone. This anatomical landmark is important in oral and maxillofacial surgery as it provides access to the pterygopalatine fossa, which contains important neurovascular structures including the maxillary artery and pterygopalatine ganglion. During maxillary down-fracture procedures in Le Fort I osteotomy, the pterygomaxillary fissure must be separated using an osteotome or pterygoid chisel to allow complete mobilization of the maxilla. The fissure also serves as a radiographic landmark for identifying fractures and planning surgical approaches to the middle third of the face.
