Oral epithelial dysplasia (OED) is a microscopic diagnosis made on tissue biopsy and is characterised by cytological and architectural abnormalities of the oral mucosal epithelium, including basal cell hyperplasia, loss of polarity, increased mitoses, and abnormal keratinisation. It is graded histologically as mild, moderate, or severe (or as low-grade/high-grade in binary systems), with higher grades correlating with elevated malignant transformation risk. Editors should note that dysplasia is a histopathological, not clinical, term and should not be applied without reference to biopsy findings. The WHO classification system is the most widely cited grading authority and should be referenced in academic documents.
