The Radiographic Union Scale for Tibial Fractures (RUST) is a validated scoring system designed to provide objective assessment of fracture healing progress in tibial fractures. The scale evaluates cortical bridging and fracture line visibility in four cortical regions (anterior, posterior, medial, and lateral) on anteroposterior and lateral radiographs. Each cortex is scored from 1 (no callus formation) to 3 (bridging callus), with total scores ranging from 4 to 12. RUST scores of 9 or higher typically indicate clinical and radiographic union. This standardized approach improves inter-observer reliability in fracture healing assessment and helps guide clinical decision-making regarding weight-bearing progression and hardware removal. The scale is particularly valuable in research studies and clinical trials evaluating fracture healing interventions.
