A compression fracture of the posterior humeral head occurring when it impacts against the glenoid rim during shoulder dislocation.

A Hill-Sachs lesion is an osteochondral impaction fracture that occurs on the posterior aspect of the humeral head as a result of anterior shoulder dislocation. Named after radiologist Harold Hill and orthopedic surgeon Maurice Sachs, this lesion forms when the soft posterior humeral head impacts against the firm anterior glenoid rim during the dislocation event. The resulting depression or 'dent' in the bone can contribute to recurrent shoulder instability if large enough. Hill-Sachs lesions are found in approximately 80% of recurrent anterior dislocations and may require surgical treatment in cases where they contribute to ongoing instability.
